Responsibilities & Requirements
Responsibilities:
- Responsible for clinical and pharmacotherapy services in the Intensive Care Unit
- Clarify, monitor, and evaluate prescribed therapies to promote optimal drug use and evidence-based practices.
- Identify and compile metrics and meaningful clinical outcomes to show impact of services
- Attend multidisciplinary rounds
- Serve as medication information resource for caregivers, patients and families
- Lead didactic sessions on ICU topics with medical residents and students upon request
- Precept pharmacy students and PGY1 residents on rotation(s) which includes prioritizing and managing work assignments
- Attend and participate in Code Blue situations in Intensive Care Unit
- Committee participation
Position Accountabilities
- The ICU Clinical Pharmacist works in a multidisciplinary environment alongside medical residents, attending physicians, nurses and auxiliary medical staff in the Intensive Care Units. The position provides pharmacotherapy services, executes medication profile review, responds to drug information questions, completes order verification, and provides didactic teaching of medical residents. The individual shares the responsibility for patient care outcomes, not just by providing basic dispensing functions and drug information services, but by solving patient- and drug-related problems and by making decisions regarding drug prescribing, monitoring, and drug regimen adjustments. The person works closely with other members of the Department of Pharmacy to address any needs that might arise. Active participation on various hospital committees is also expected. Opportunities to precept advanced practice pharmacy students are available.
Requirements:
- Minimum Education: B.S. in Pharmacy or PharmD from an ACPE accredited School of Pharmacy
- Required Qualifications:
- Completion of ASHP-accredited PGY1 residency OR
- Board of Pharmacy Specialties (BPS) Certification with pharmacist experience in Critical Care
- Preferred Experience:
- PGY2 in Critical Care
OR
- Board of Pharmacy Specialties (BPS) Certification with 3 years of pharmacist experience in Critical Care
- Certification/Registration: Licensed as Pharmacist in the state of Ohio
- Advanced Cardiac Life Support (ACLS)certification
- Ability to develop strong interpersonal relationships
- Flexible attitude and ability to adapt to schedule and workload changes
- Professional presentation and demeanor
- Collaboration skills
- Continue staffing requirements for weekends & Holidays as currently outlined
